this update is primarily for allison, so she can keep up with the goings on of work while she is on... spring break. haha. if only hospitals could shut down for a week in the spring, and on holidays, and weekends.
tonight has been average? i dunno i kinda feel like tonight is how nights should be. busy in the beginning, but eventually settling enough to chart and get caught up before the 5 am rush hits. dont get me wrong, i've still found tonight to be miserable, but in a less panicked, more "i wish i was sleeping or curled up with a book" sort of way.
Sidenote-- lately i have a compulsion to check the ink level in whatever my current primarily used pen is. Right now it is a Lakeside Baptist Church pen from birmingham... i'm wondering when I acquired it-- coulda been as early as the church table day in the caf freshman year. but considering how my school was, there were lots of baptist or nondenominational church pens floating around. this pen has like a centimeter and a half of ink left in it... i better pick a new one to bring for this week!! how exciting. haha.
i need to make up some like... work stretches. my neck and back hurt, and i need to be stretching my IT band while I'm here.
tonight i started with 5 patients one was an assigned admit. one was discharged and left in the middle of report, one was transferred to main campus just after report, and the admit still wasn't on the floor. that left me with 3 people to see. which was nice for that little stretch. those three were all under the age of 60 too. my admission got here at 9, i hurried to get the profile done and got percocet ordered for him. he is a bit of a weirdo. actually all of mine are. so at that point i had 4 patients, but that would mean i was going to get a LOT of admissions. luckily they rearranged one so I took 424 from Sandra, putting me at 5. i was going to get another admission, but then they never found another nurse for the morning, so they couldn't give us more patients. as it is, tracy will have 8 patients, and sosina and sebreena will have 7. yikes yikes.
i had another missing tele box incident. I'm pretty sure this is my third. when 433 left, i asked the paramedics if they had taken off our box and they said they left it at the desk. i saw one there, so i presumed it was mine. a couple hours later, the monitor tech calls wondering if 433 was in the room because it was showing a flatline. i forgot about the call until i looked at the monitor myself and was like, who is that person who looks dead? oh crap! that is the transferred patient. so i looked around the desk, no box. luckily i went to the room and housekeeping had hung it on the IV pole. thank god it didn't get wrapped up in the old linen. crisis averted.
then i have one patient on PPN, with lipids. which is weird in and of itself. then one of the IVs infiltrated a little bit (i took it out and PPN leaked out of his skin. sick). then like two hours later, the IV where I had switched the PPN to started burning. then it stopped flushing. sigh. Keisha came over from ICU and started a new one, where i had to run some antibiotics. but then i also had to restart the ppn in it... desperate times. (I got some help in making that call and i flushed it a lot beforehand, i know you aren't supposed to run ANYTHING in the same line) anyway. he's gonna need a new line for antibiotics and dilaudid. but i'm not going to put it in.
I think my admission has the beginnings of myxedema-- elevated TSH (12.0), low b/p, came in with hypoglycemia, skin tight with swelling, thin hair. I felt like quite the nursing nancy drew for putting all that together. go me.
ok that is plenty for now. 5 am has hit, i have antibiotics to give and some labs to draw.
